WHAT THIS MEDICINE IS USED FOR

• Controlling high blood sugar levels in type 2 diabetes
• Used along with diet and exercise as advised by your doctor
• Sometimes used along with other diabetes medicines or insulin

HOW IT WORKS

• Dapagliflozin helps the kidneys remove extra sugar from the body through urine
• Sitagliptin helps the body release more insulin and lowers sugar production by the liver after meals
• Metformin reduces sugar production in the liver and improves how the body uses insulin

• Not used for type 1 diabetes

• Signs of kidney problems: reduced urination, swelling in legs

HOW TO TAKE IT

• Take exactly as prescribed by your doctor, usually once daily with the first main meal of the day
• Swallow the tablet whole with water; do not crush or chew unless your doctor says it is safe
• Drink enough water during the day, especially in hot weather
• Continue to follow the diet and exercise plan advised by your doctor
• Do not stop the medicine suddenly without consulting your doctor, even if you feel well

IF YOU MISS A DOSE

• Take it as soon as you remember on the same day
• If it is almost time for the next dose, skip the missed dose
• Do not take a double dose to make up for the missed one
• If you miss doses often, inform your doctor

DO NOT TAKE THIS MEDICINE IF YOU

• Are allergic to dapagliflozin, sitagliptin, metformin, or any ingredient in the tablet
• Have severe kidney disease or are on dialysis
• Have diabetic ketoacidosis or type 1 diabetes
• Have severe liver disease
• Have a serious infection, dehydration, or are having major surgery soon

TELL YOUR DOCTOR BEFORE STARTING IF YOU

• Have a history of urinary tract or genital infections
• Have heart failure or bladder problems
• Have a history of pancreatitis
• Are pregnant, planning pregnancy, or breastfeeding
