WHAT THIS MEDICINE IS USED FOR

• Controlling high blood sugar levels in type 2 diabetes

HOW IT WORKS

• Dapagliflozin helps kidneys remove extra sugar through urine

• Severe dehydration or very low blood pressure
• Signs of ketoacidosis even with normal blood sugar, such as nausea, vomiting, stomach pain
• Severe allergic reaction with swelling of face, lips or throat
• Signs of pancreatitis such as severe stomach pain radiating to the back
• Sudden decrease in urination or signs of kidney problems

HOW TO TAKE IT

• Take exactly as prescribed by your doctor, usually with meals
• Swallow the tablet whole with water; do not crush or chew
• Do not change the dose or stop without consulting your doctor
• Continue to follow your diet and exercise plan
• Monitor blood sugar regularly as advised

IF YOU MISS A DOSE

• Take it as soon as you remember
• If it is almost time for your next dose, skip the missed one
• Do not take a double dose to make up for a missed one

DO NOT TAKE THIS MEDICINE IF YOU

• Are allergic to dapagliflozin, sitagliptin, metformin or any ingredient
• Have type 1 diabetes or diabetic ketoacidosis
• Have severe kidney disease
• Have severe liver disease
• Are undergoing dialysis

TELL YOUR DOCTOR BEFORE STARTING IF YOU

• Have any kidney or liver problems
• Have a history of pancreatitis
• Have recurrent urinary or genital infections
• Are dehydrated or have low blood pressure
• Are pregnant, planning pregnancy, or breastfeeding
• Are scheduled for surgery or an imaging test involving contrast dye
• Drink alcohol regularly
