This combination medicine is used to manage high blood sugar levels in adults with type 2 diabetes mellitus.

WHAT THIS MEDICINE IS USED FOR

• To control blood sugar levels in type 2 diabetes
• Used along with diet and exercise for better sugar control
• Prescribed when a single diabetes medicine is not enough

HOW IT WORKS

• Sitagliptin helps the body release more insulin after meals and lowers sugar production by the liver
• Glimepiride stimulates the pancreas to release more insulin
• Metformin reduces sugar production in the liver and improves how the body uses insulin

WHAT IT IS NOT USED FOR

• Not used for type 1 diabetes
• Not used for diabetic ketoacidosis, a serious emergency condition

SERIOUS SIDE EFFECTS — STOP AND SEE A DOCTOR IMMEDIATELY

• Signs of severe low blood sugar such as confusion, blurred vision, fainting
• Signs of lactic acidosis such as muscle pain, difficulty breathing, unusual tiredness, stomach pain
• Severe stomach pain that may spread to the back, with or without vomiting (possible pancreatitis)
• Yellowing of skin or eyes, dark urine (liver problems)
• Severe allergic reaction with swelling of face, lips, throat or difficulty breathing
• Unusual bruising or bleeding

HOW TO TAKE IT

• Take this medicine exactly as prescribed by your doctor, usually with meals to reduce stomach upset
• Swallow the tablet whole with water; do not crush or chew it
• Do not change the dose or stop the medicine on your own
• Continue to follow the diet and exercise plan advised by your doctor
• Get your blood sugar and kidney function checked regularly as advised

IF YOU MISS A DOSE

• Take the missed dose as soon as you remember
• If it is almost time for the next dose, skip the missed one and continue your normal schedule
• Do not take two doses together to make up for a missed one
• Carry a fast-acting sugar source in case of low blood sugar symptoms

• Store at room temperature, away from heat and moisture

DO NOT TAKE THIS MEDICINE IF YOU

• Are allergic to sitagliptin, glimepiride, metformin or any other ingredient in this medicine
• Have type 1 diabetes or diabetic ketoacidosis
• Have severe kidney or liver disease
• Have severe heart failure or a recent heart attack
• Are pregnant or breastfeeding, unless advised by your doctor

TELL YOUR DOCTOR BEFORE STARTING IF YOU

• Have any kidney, liver or heart problems
• Have a history of pancreatitis
• Drink alcohol regularly
• Are scheduled for surgery or an X-ray procedure using contrast dye
• Have a history of low blood sugar episodes
• Are elderly or have poor nutrition

• Steroids and certain blood pressure medicines
• Diuretics (water pills)
• Contrast dye used in imaging tests
• Alcohol
