This combination medicine is used to treat type 2 diabetes mellitus, especially when diet, exercise and a single medicine alone are not enough to control blood sugar levels.

WHAT THIS MEDICINE IS USED FOR

• Controlling high blood sugar in adults with type 2 diabetes
• Used along with diet and exercise for better blood sugar control
• Sometimes used with other diabetes medicines as advised by the doctor

HOW IT WORKS

• Empagliflozin blocks a kidney protein (SGLT2) so extra sugar is passed out in urine
• Metformin lowers sugar production by the liver and helps muscles use sugar better
• Together they lower blood sugar through two different actions

WHAT IT IS NOT USED FOR

• Not used for type 1 diabetes
• Not used for diabetic ketoacidosis
• Not a treatment for weight loss, though some weight loss may occur

• Signs of low blood sugar such as sweating, shakiness, confusion
• Signs of ketoacidosis such as nausea, vomiting, stomach pain, fast breathing even with normal sugar levels
• Signs of dehydration or low blood pressure such as dizziness on standing
• Severe urinary or genital infection with fever
• Signs of lactic acidosis such as muscle pain, difficulty breathing, extreme tiredness
• Sudden swelling of face, lips or throat, or difficulty breathing
• Dark urine or yellowing of skin/eyes

HOW TO TAKE IT

• Take exactly as prescribed by your doctor, usually with food to reduce stomach upset
• Swallow the tablet whole with water; do not crush or chew unless advised
• Take at the same time(s) each day for best results
• Continue diet and exercise measures advised by your doctor
• Do not stop the medicine suddenly without consulting your doctor

IF YOU MISS A DOSE

• Take it as soon as you remember
• If it is almost time for the next dose, skip the missed dose
• Do not take a double dose to make up for a missed one
• If you miss doses often, tell your doctor

STORAGE

• Store at room temperature, away from direct heat, light and moisture
• Keep in the original pack until it is time to take the medicine

DO NOT TAKE THIS MEDICINE IF YOU

• Are allergic to empagliflozin, metformin or any ingredient in the tablet
• Have severe kidney disease
• Have diabetic ketoacidosis
• Have severe liver disease
• Have a serious infection, severe dehydration or are going for major surgery
• Have a history of lactic acidosis

TELL YOUR DOCTOR BEFORE STARTING IF YOU

• Have kidney or liver problems
• Have a history of urinary or genital infections
• Are pregnant, planning pregnancy, or breastfeeding
• Drink alcohol regularly
• Are elderly or have heart disease
• Are scheduled for an X-ray test involving contrast dye
• Follow a low-carbohydrate or ketogenic diet
