This medicine is used to manage type 2 diabetes mellitus, especially when diet, exercise and a single diabetes medicine are not enough to control blood sugar levels.

WHAT THIS MEDICINE IS USED FOR

• Controlling high blood sugar in type 2 diabetes
• Used when metformin alone does not control sugar levels adequately
• Helps lower risk of long-term diabetes complications when blood sugar is controlled

HOW IT WORKS

• Glibenclamide stimulates the pancreas to release more insulin
• Metformin decreases glucose production by the liver
• Metformin also improves how muscles and tissues use insulin
• Together they lower blood sugar through two different mechanisms

WHAT IT IS NOT USED FOR

• Not used for type 1 diabetes

COMMON SIDE EFFECTS

SERIOUS SIDE EFFECTS — STOP AND SEE A DOCTOR IMMEDIATELY

• Severe hypoglycemia with confusion, sweating, shakiness, or loss of consciousness
• Signs of lactic acidosis such as unusual tiredness, muscle pain, difficulty breathing, stomach pain with vomiting
• Yellowing of skin or eyes (jaundice)
• Severe allergic reaction with swelling of face, lips or throat, or difficulty breathing
• Dark urine or pale stools

How to Use GLUCORED FORTE TABLET (GLIBENCLAMIDE 5MG + METFORMIN 500MG)

HOW TO TAKE IT

• Take exactly as prescribed by your doctor, usually with breakfast or the first main meal
• Swallow the tablet with water; do not crush or chew unless advised
• Do not skip meals after taking this medicine as it can cause low blood sugar
• Continue to follow diet and exercise advice given by your doctor
• Do not increase or decrease the dose on your own

IF YOU MISS A DOSE

• Take it as soon as you remember with food
• If it is almost time for your next dose, skip the missed one
• Do not take two doses together to make up for a missed one
• If you frequently miss doses, discuss a reminder plan with your doctor

STORAGE

• Store at room temperature, away from heat and direct sunlight
• Keep in a dry place, away from moisture
• Keep out of reach of children

DO NOT TAKE THIS MEDICINE IF YOU
