This medicine is used to manage type 2 diabetes mellitus in adults, usually when diet, exercise and single medicines are not enough to control blood sugar.

WHAT THIS MEDICINE IS USED FOR

• Controlling high blood sugar levels in type 2 diabetes
• Used along with diet and exercise
• Sometimes used with other diabetes medicines as advised by the doctor

HOW IT WORKS

• Empagliflozin helps the kidneys remove excess sugar through urine
• Sitagliptin increases natural insulin release after meals and reduces sugar production by the liver

• Swelling of face, lips, tongue or throat, or severe skin rash (allergic reaction)
• Signs of ketoacidosis even with normal blood sugar (fruity breath, vomiting, weakness)
• Sudden decrease in urination or swelling of legs (kidney problem)

HOW TO TAKE IT

• Take exactly as prescribed by your doctor, usually with a meal to reduce stomach upset
• Swallow the tablet whole with water; do not crush or chew
• Take it at the same time(s) each day for best results
• Continue taking it regularly even if you feel well; diabetes medicines work best with consistent use
• Do not stop or change the dose without consulting your doctor
• Follow the diet and exercise plan advised by your doctor along with this medicine

IF YOU MISS A DOSE

• Take it as soon as you remember
• If it is almost time for your next dose, skip the missed dose
• Do not take two doses together to make up for a missed one

DO NOT TAKE THIS MEDICINE IF YOU

• Are allergic to empagliflozin, sitagliptin, metformin or any ingredient in this medicine
• Have type 1 diabetes or diabetic ketoacidosis
• Have severe kidney disease or are on dialysis
• Have severe liver disease
• Have a history of metabolic acidosis

TELL YOUR DOCTOR BEFORE STARTING IF YOU

• Have kidney or liver problems
• Have a history of urinary tract or genital infections
• Are prone to low blood pressure or dehydration
• Have a history of pancreatitis
• Are pregnant, planning pregnancy, or breastfeeding
• Are scheduled for surgery or an imaging test involving contrast dye
• Drink alcohol regularly
