WHAT THIS MEDICINE IS USED FOR

• Controlling high blood sugar (hyperglycemia) in type 2 diabetes
• Used alongside diet and exercise, not as a replacement for them

HOW IT WORKS

• Dapagliflozin helps the kidneys remove excess sugar through urine

• Sudden swelling of face, lips or throat, or difficulty breathing (allergic reaction)
• Signs of dehydration or very low blood pressure, such as fainting

HOW TO TAKE IT

• Take exactly as prescribed by your doctor, usually with a meal to reduce stomach upset
• Swallow the tablet whole with water; do not crush or chew unless your doctor says so
• Take it at the same time(s) each day for consistent blood sugar control
• Continue taking it regularly even if you feel well; diabetes control needs ongoing treatment

• Drink enough water through the day as this medicine increases urination

IF YOU MISS A DOSE

• Take it as soon as you remember on the same day

DO NOT TAKE THIS MEDICINE IF YOU

• Are allergic to Dapagliflozin, Sitagliptin, Metformin or any ingredient in the tablet
• Have severe kidney disease
• Have diabetic ketoacidosis or a history of it
• Have severe liver disease
• Have a condition causing low oxygen or severe dehydration
• Are going to undergo certain scans involving contrast dye (tell your doctor in advance)
