Uses and Benefits of STALIX-M 50/1000MG TABLETS (SITAGLIPTIN 50MG+METFORMIN 1000MG)

Sitagliptin 50mg + Metformin Hydrochloride 1000mg is a fixed-dose combination (FDC) tablet prescribed for the long-term control of type 2 diabetes mellitus in adults, along with a diabetic diet and regular physical activity. This is the higher metformin strength of the combination, usually given when a lower dose no longer keeps blood sugar in the target range.

WHAT THIS TABLET IS USED FOR

• Lowering high blood sugar in adults with type 2 diabetes when metformin alone, or diet and exercise alone, are not enough to reach the HbA1c target.
• Controlling fasting blood sugar as well as post-meal (post-prandial) sugar spikes with one tablet instead of two separate medicines — fewer pills means better daily compliance.
• Continuing treatment in patients already taking sitagliptin and metformin separately, switched to a single combination tablet.
• As part of a larger treatment plan along with a sulfonylurea (glimepiride, gliclazide) or insulin, when your doctor decides it is needed.

HOW IT WORKS

This tablet combines two medicines that lower blood sugar by two completely different routes, which is why the combination works better than either drug alone:

• Sitagliptin (DPP-4 inhibitor) blocks the DPP-4 enzyme, which increases the level of natural incretin hormones (GLP-1 and GIP) in your body. This makes the pancreas release more insulin only when blood sugar is high, and at the same time reduces the release of glucagon, the hormone that tells the liver to push out more sugar. Because the effect is glucose-dependent, sitagliptin on its own carries a low risk of hypoglycaemia.
• Metformin (biguanide) reduces the amount of glucose your liver produces, slows down glucose absorption from the intestine, and makes your muscle and fat cells more sensitive to your own insulin. Metformin is the first-line medicine for type 2 diabetes worldwide and is weight-neutral for most people.

WHY GOOD SUGAR CONTROL MATTERS

Keeping HbA1c within the range your doctor has set, over years and not just weeks, is what actually reduces the risk of the long-term complications of diabetes — kidney damage (diabetic nephropathy), nerve damage (neuropathy) with numbness and tingling in the feet, eye damage (diabetic retinopathy), non-healing foot ulcers, and heart and blood-vessel disease. This tablet controls diabetes; it does not cure it, so it is normally taken long term.

WHAT IT IS NOT USED FOR

• Type 1 diabetes (insulin-dependent diabetes).
• Diabetic ketoacidosis or diabetic coma — these are emergencies that need hospital treatment.

Side Effects of STALIX-M 50/1000MG TABLETS (SITAGLIPTIN 50MG+METFORMIN 1000MG)

Most people tolerate Sitagliptin 50mg + Metformin 1000mg well. Many of the side effects are related to metformin, are strongest in the first 1–2 weeks, and settle down on their own as the body adjusts — especially if the tablet is taken with food.

COMMON SIDE EFFECTS (usually mild, often temporary)

• Nausea and vomiting
• Diarrhoea or loose motions
• Stomach pain, bloating, gas (flatulence)
• Loss of appetite
• A metallic taste in the mouth
• Headache
• Blocked or runny nose, sore throat, upper respiratory infection

LESS COMMON SIDE EFFECTS

• Low blood sugar (hypoglycaemia) — uncommon with this combination alone, but the risk goes up clearly if you are also taking a sulfonylurea or insulin, if you skip meals, fast, or exercise much more than usual. Warning signs are sweating, shakiness, palpitations, sudden hunger, confusion, dizziness or blurred vision. Take glucose, sugar, juice or 3–4 sweets immediately and inform your doctor.
• Vitamin B12 deficiency with long-term metformin use, which can cause tiredness, weakness, or tingling in the hands and feet. Your doctor may check B12 levels periodically and prescribe a supplement.
• Constipation, indigestion or heartburn
• Skin rash or itching
• Dizziness or drowsiness

SERIOUS SIDE EFFECTS — STOP THE TABLET AND SEE A DOCTOR IMMEDIATELY

• Lactic acidosis (rare but a medical emergency, linked to metformin): unusual muscle pain or weakness, difficulty breathing, unusual sleepiness, severe stomach pain with nausea and vomiting, feeling cold, or a slow or irregular heartbeat. Risk is higher in kidney disease, dehydration, severe infection, heart failure or heavy alcohol use.
• Pancreatitis (linked to sitagliptin): severe, continuous pain in the upper stomach that spreads to the back, with or without vomiting.
• Severe and disabling joint pain, which can start any time from a day to years after starting a DPP-4 inhibitor.
• Bullous pemphigoid — blisters or peeling of the skin.
• Signs of an allergic reaction: swelling of the face, lips, tongue or throat, difficulty breathing, or widespread rash.
• Signs of liver problems: yellowing of the eyes or skin, dark urine, or persistent right-sided upper abdominal pain.

How to Use STALIX-M 50/1000MG TABLETS (SITAGLIPTIN 50MG+METFORMIN 1000MG)

Take this medicine exactly as your doctor has prescribed. The dose depends on your blood sugar readings, HbA1c, kidney function and the other diabetes medicines you are taking, so never copy someone else's prescription.

HOW TO TAKE IT

• Swallow the tablet whole with a glass of water. Do not crush, chew, break or dissolve it.
• Always take it with food or immediately after a meal. Taking it on an empty stomach is the single most common reason for nausea, loose motions and stomach pain with this combination.
• The 50mg + 1000mg strength is most often prescribed twice a day — one tablet with breakfast and one with dinner — which gives the standard maximum of 100mg sitagliptin and 2000mg metformin per day. Follow the timing written on your prescription.
• Take it at the same times every day. Steady timing gives steadier sugar control.
• Do not take more than one tablet at a time and do not exceed 100mg of sitagliptin in 24 hours from all sources.

IF YOU MISS A DOSE

Take it as soon as you remember, with some food. If it is almost time for your next dose, skip the missed one and continue as usual. Never take two tablets together to make up for a missed dose.

WHILE YOU ARE ON THIS MEDICINE

• Do not stop the tablet on your own, even if you feel completely fine — blood sugar usually has no symptoms until damage is already done.
• Keep monitoring fasting and post-meal sugar at home as advised, and get HbA1c checked about every 3 months until stable.
• Get kidney function (serum creatinine / eGFR) checked before starting and at least once a year, more often if you are elderly or have kidney disease.
• Keep up the diet plan and at least 30 minutes of activity most days. Medicines work far better alongside lifestyle changes than in place of them.
• Always keep glucose, sugar or sweets with you in case of a hypoglycaemia episode.

STORAGE

Store below 25–30°C in a dry place, away from direct sunlight and moisture. Keep it in the original strip or bottle, out of the reach of children. Do not use after the expiry date printed on the pack.

Sitagliptin + Metformin is a prescription-only (Schedule H) medicine. Use it only under a doctor's supervision.

DO NOT TAKE THIS MEDICINE IF YOU

• Have severe kidney disease or reduced kidney function (generally eGFR below 30 ml/min).
• Have diabetic ketoacidosis or diabetic pre-coma.
• Have severe liver disease.
• Have had an allergic reaction to sitagliptin, metformin or any ingredient in the tablet.
• Have a condition causing low oxygen or poor circulation — severe infection, dehydration, shock, recent heart attack, or decompensated heart failure.
• Have type 1 diabetes.

TELL YOUR DOCTOR BEFORE STARTING IF YOU

• Have any kidney or liver problem, or a history of pancreatitis or gallstones.
• Have heart failure or a heart condition.
• Are over 65 years of age, or eat and drink very little.
• Drink alcohol regularly or heavily — alcohol raises the risk of both lactic acidosis and low blood sugar.
• Are pregnant, planning a pregnancy, or breastfeeding. This combination is generally not recommended in pregnancy; diabetes in pregnancy is usually managed with insulin.

TEMPORARY STOPS YOU MUST KNOW ABOUT

• Before any X-ray, CT scan or angiography using iodine-based contrast dye, and before surgery under general or spinal anaesthesia, metformin is usually stopped and restarted only 48 hours later once kidney function is confirmed to be normal. Always tell the radiologist, surgeon or anaesthetist that you are on metformin.
• During severe vomiting, diarrhoea, high fever or any illness where you cannot eat or drink normally, contact your doctor — dehydration is the main trigger for lactic acidosis.

MEDICINES THAT CAN INTERACT

• Sulfonylureas (glimepiride, gliclazide) and insulin — increase the risk of hypoglycaemia; your doctor may lower their dose.
• Diuretics, steroids (prednisolone), thyroid medicines and some blood pressure medicines can raise blood sugar.
• NSAID painkillers, ACE inhibitors and ARBs can affect kidney function and therefore metformin safety.
• Digoxin, ranolazine, cimetidine and certain other drugs can change metformin levels in the blood.

ALCOHOL, DRIVING AND DAILY LIFE

Avoid alcohol, especially on an empty stomach. This tablet on its own is unlikely to affect driving, but if it is combined with a sulfonylurea or insulin, do not drive or operate machinery when you feel the warning signs of low blood sugar.
