This medicine is used mainly for the treatment of high blood pressure (hypertension) in adults who need more than one medicine to control their blood pressure.
WHAT THIS MEDICINE IS USED FOR
- Long-term control of high blood pressure
- Reducing the risk of heart attack, stroke and kidney problems linked to uncontrolled blood pressure
- Use in patients whose blood pressure is not controlled by a single blood pressure medicine
HOW IT WORKS
- Telmisartan blocks a hormone called angiotensin II, relaxing and widening blood vessels
- Amlodipine is a calcium channel blocker that relaxes the muscles of blood vessel walls
- Chlorthalidone is a diuretic (water pill) that helps the kidneys remove extra salt and water from the body
- Together, these three actions lower blood pressure more effectively than a single medicine
WHAT IT IS NOT USED FOR
- Not used for immediate emergency lowering of dangerously high blood pressure
- Not a treatment for chest pain (angina) occurring suddenly
- Not used for conditions unrelated to blood pressure, such as infections or pain
- Not recommended for initial therapy unless your doctor decides combination treatment is needed from the start

- Severe dizziness or fainting
- Very low urine output or signs of kidney problems
- Irregular heartbeat, muscle weakness or cramps (may indicate low potassium or sodium levels)
- Yellowing of skin or eyes (signs of liver problems)
- Severe skin rash or blistering

HOW TO TAKE IT
- Take this medicine exactly as prescribed by your doctor, usually once a day
- It can be taken with or without food, but try to take it at the same time every day
- Swallow the tablet with a glass of water; do not crush or chew unless advised
- Continue taking it regularly even if you feel well, as blood pressure control needs consistent use
- Do not stop the medicine suddenly without talking to your doctor
IF YOU MISS A DOSE
- Take the missed dose as soon as you remember on the same day
- If it is almost time for your next dose, skip the missed one and continue your normal schedule
- Do not take two doses together to make up for a missed one
STORAGE
- Store at room temperature, away from direct sunlight and moisture

DO NOT TAKE THIS MEDICINE IF YOU

- Have a known allergy to Telmisartan, Amlodipine, Chlorthalidone, or sulfonamide-based medicines
- Have severe low blood pressure or unstable heart conditions

- Have diabetes, gout, or a history of low potassium or sodium levels
- Have any heart valve problems or recent heart attack
- Have liver or kidney disease of any severity
