Warfarin is used to prevent and treat conditions caused by unwanted blood clots forming in the veins, arteries, heart, or lungs.

WHAT THIS MEDICINE IS USED FOR

• Preventing and treating deep vein thrombosis (DVT)
• Preventing and treating pulmonary embolism (clots in the lungs)
• Preventing stroke in people with irregular heart rhythm (atrial fibrillation)
• Preventing clots in people with artificial heart valves
• Preventing clots after certain heart attacks

HOW IT WORKS

• Blocks vitamin K-dependent clotting factors made by the liver
• Slows down the blood's ability to form clots
• Helps keep blood flowing smoothly through veins and arteries
• Takes several days to reach full effect, so doses are adjusted gradually

WHAT IT IS NOT USED FOR

• Not used for immediate pain relief
• Not a treatment for infections
• Not used to dissolve an existing large clot quickly (other medicines are used for that)
• Not for use without regular blood monitoring

• Vomiting blood or coffee-ground material
• Severe or unexplained bruising
• Heavy or prolonged bleeding from any site
• Sudden severe headache, weakness, or confusion (signs of bleeding in the brain)
• Purple, painful skin patches (a rare skin reaction)
• Yellowing of skin or eyes

How to Use WARF 4MG TABLET (WARFARIN 4MG)

HOW TO TAKE IT

• Take exactly as prescribed by your doctor, usually once a day
• Take it at the same time each day, often in the evening
• Can be taken with or without food
• Do not change your dose on your own, even if you feel fine
• Keep your diet consistent, especially foods rich in vitamin K like leafy greens, as sudden changes can affect how the medicine works
• Attend all scheduled blood tests (INR) to check the medicine is working safely

IF YOU MISS A DOSE

• Take it as soon as you remember on the same day
• If you remember the next day, skip the missed dose and continue as usual
• Never take a double dose to make up for a missed one
• Inform your doctor if you miss doses often

STORAGE

• Store at room temperature, away from heat and direct sunlight
• Keep in the original container, away from moisture
• Keep out of reach of children
• Do not use after the expiry date printed on the pack

DO NOT TAKE THIS MEDICINE IF YOU

• Are pregnant (can harm the baby) or planning pregnancy
• Have active bleeding or a bleeding disorder
• Have severe liver or kidney disease
• Are allergic to warfarin or any of its ingredients
• Have recently had major surgery or are at high risk of bleeding
